Put these points in your notice

which of your works is affected (several works can be listed together);
the precise page address on our site;
your full name plus email, postal address or phone number;

your declaration, under penalty of perjury, that the notice is accurate and that you own the right or may act on behalf of the owner;
your physical or electronic signature (a typed full name is enough).

After it arrives

A complete notice leads to removal or blocking of the material, and we confirm once it is done. The person who posted it may answer with a counter-notice containing the same kind of details and agreeing to the jurisdiction of the federal district court for their address. If the rights holder has not told us about a lawsuit after 10–14 working days, we may put the material back.

False claims

Under 17 U.S.C. 512(f), anyone who knowingly misrepresents infringement can be held liable for the resulting damages. Check the facts first.
